The Adult Evaluation Reviews Program provides assessments (an exploration of social, medical, financial, functional and emotional status), develops care plans and coordinates services on the population considered at risk of institutionalization. Services are coordinated with the psychiatrist and psychologist on all pre-admission screens and resident review nursing home clients.

The goal of these services is to help people remain in the community or in the least restrictive living situation where they can function at the highest level of independence and personal well-being.

Services include physical therapy, occupational therapy, speech therapy, medical social work, home health aide, bath aide, phlebotomy, medication management and more.

The Chesapeake-Potomac Health Agency is a nonprofit agency owned by the alliance of CalvertHealth Medical Center, the University of Maryland Charles Regional Medical Center, MedStar St. Mary's Hospital and the St. Mary's Health Department.
