The Friendly Health Services Adult Day Care team assesses each individual's needs in collaboration with family members and the personal physician. A therapeutic plan of care is developed and reviewed every 90 days, considering health status, mental health, nutrition, rehabilitation needs, counseling, recreation, and social needs. Family involvement is essential to the center’s approach. From the time of admission, family members contribute to care planning and the ongoing evaluation of the program. Care plan meetings are held periodically throughout the year, with family members, caregivers, and other service staff invited to participate.
